    1. Understand you are Enrolling your child In The 2025 Wave of Hope Learning Center Imagination Station Summer Camp. By agreeing to the Summer Camp Code of Conduct and Waiver, and that you must pay $150 per week of each camp choice selected.

  • 2025 WAVE OF HOPE LEARNING CENTER IMAGINATION STATION
    SUMMER CAMP WAIVER AND RELEASE OF LIABILITY
    IN CONSIDERATION OF the risk of injury that exists while participating in 2025 WAVE OF HOPE LEARNING CENTER IMAGINATION STATION SUMMER CAMP ACTIVITIES (hereinafter the "Activity"); and
    IN CONSIDERATION OF my desire to participate in said Activity and being given the right to participate in same;
    I HEREBY, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, "Releasor," "I" or "me", which terms shall also include Releasor's parents or guardian if Releasor is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in the Activity; and
    I HEREBY release and forever discharge WAVE OF HOPE MINISTRY AND THE PROPERTY OWNERS, located at 1111 6th St, Greeley, Colorado 80631, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns (collectively "Releasees"), from any physical or psychological injury that I may suffer as a direct result of my participation in the aforementioned Activity.
    I AM ALLOWING MY CHILD TO VOLUNTARILY PARTICIPATE IN THE AFOREMENTIONED ACTIVITIES AT THEIR OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH PARTICIPATING IN THE ACTIVITIES, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO: PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR EMOTIONAL LOSS, AND DEATH. I UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS' NEGLIGENCE, CONDITIONS RELATED TO TRAVEL TO AND FROM THE ACTIVITY, OR FROM CONDITIONS AT THE ACTIVITY LOCATION(S). NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH KNOWN AND UNKNOWN TO ME, OF MY PARTICIPATION IN THIS ACTIVITY.
    I FURTHER AGREE to indemnify, defend and hold harmless the Releasees against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs.
    I FURTHER ACKNOWLEDGE that Releasees are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of Releasees. In the event that I should require medical care or treatment, I authorize Wave of Hope Ministry to provide all emergency medical care deemed necessary, including but not limited to, first aid, CPR, the use of AEDs, emergency medical transport, and sharing of medical information with medical personnel. I further agree to assume all costs involved and agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance.

    I FURTHER ACKNOWLEDGE that Activities may involve a test of a person's physical and mental limits and may carry with it the potential for death, serious injury, and property loss. I agree that it is the decision of the Wave of Hope Ministry official or agent, regarding their approval for my child to participate in an Activity.
    I HEREBY ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS "WAIVER AND RELEASE" AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. I EXPRESSLY AGREE TO RELEASE AND DISCHARGE Wave of Hope Ministry and the Salvation Army AND ALL OF ITS AFFILIATES, MANAGERS, MEMBERS, AGENTS, ATTORNEYS, STAFF, VOLUNTEERS, HEIRS, REPRESENTATIVES, PREDECESSORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL CLAIMS OR CAUSES OF ACTION AND I AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANY RIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL ACTION AGAINST WAVE OF HOPE MINISTRY OR SALVATION ARMY STAFF, ASSOCIATES, EMPLOYEES OR AFFILIATES FOR PERSONAL INJURY OR PROPERTY DAMAGE.
    To the extent that statute or case law does not prohibit releasees for ordinary negligence, this release is also for such negligence on the part of Wave of Hope Ministry and the Salvation Army, its agents, and employees.
    I agree that this Release shall be governed for all purposes by Colorado law, without regard to any conflict of law principles. This Release supersedes any and all previous oral or written promises or other agreements.
    In the event that any damage to equipment or facilities occurs as a result of my or my family's or my agent's willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any such actions of neglect or recklessness.
    THIS WAIVER AND RELEASE OF LIABILITY SHALL REMAIN IN EFFECT FOR THE DURATION OF MY PARTICIPATION IN THE ACTIVITY, DURING THIS INITIAL AND ALL SUBSEQUENT EVENTS OF PARTICIPATION.
    THIS AGREEMENT was entered into at arm's-length, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. This is an agreement between myself as the parent/guardian of the registered child and Wave of Hope Ministry and the Salvation Army and that I agree that this agreement is clear and unambiguous as to its terms, and that no other evidence shall be used or admitted to alter or explain the terms of this agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into.
    In the event that any provision contained within this Release of Liability shall be deemed to be severable or invalid, or if any term, condition, phrase or portion of this agreement shall be determined to be unlawful or otherwise unenforceable, the remainder of this agreement shall remain in full force and effect. If a court should find that any provision of this agreement to be invalid or unenforceable, but that by limiting said provision it would become valid and enforceable, then said provision shall be deemed to be written, construed and enforced as so limited.

    *I, as Parent/Guardian, have read and understand the Code of Conduct and my registered child and I agree to abide by its rules. I also understand that if I or they do not abide by these rules the consequences listed will be enforced.

    *I, parent/guardian of the registered youth, have read and have assisted my child in understanding the above rules, and requirements as a camp participant. We both understand the consequences that will follow as listed above if my child does not abide by the rules.

    The following guidelines are designed to make their experience at the 2025 Wave of Hope Imagination Station Summer Camp satisfying to them and to all others attending. The individual rights, safety and property of others must be respected.

    - Campers must bring their own cold lunch, midmorning and midafternoon snacks as well as their own labeled water bottle. Please ensure that they understand that they should not/cannot share food with other campers due to possible food allergies.

    1. Respect the rights and property of others.
    - Do not touch other people’s belongings.
    - Disrespectful, abusive language will not be a part of camp (No profanity, racial slurs,
    or putdowns).
    - Do not damage or deface facilities or property.
    - Rudeness, lack of courtesy, and disrespect for authority will not be tolerated.
    - Fighting and threatening physical abuse is not acceptable behavior.
    - All clothing worn shall be within the bounds of decency and in good taste. No halter tops, tube tops or bare backs or shirts that show cleavage or excessively baggy or tight fitting clothes will be acceptable.
    - Items of clothing which display profanity; advertises gang affiliation or products or slogans which promote tobacco, alcohol or drugs or are in any way distracting, will not be allowed.
    -Campers may NOT touch kitchen materials or appliances (stove, microwave, Keurig, etc.) without the direction of their Camp Leader or Leader’s assistant.

    2. Be concerned for the safety of campers and staff.
    - No running in the building unless during an organized activity.
    - Must wear closed-toe shoes for camp activities. Sandals are not safe on uneven terrain.
    - Do not litter, throw all trash away in trash cans.
    - Throwing objects is not allowed unless it is a planned activity such as sports (Throwing rocks, bark, etc. will not be tolerated).
    - Campers cannot leave the grounds without an adult. Adult staff must also have the Executive Director’s permission
    - All prescriptions and over the counter drugs must be given to the Site Director immediately upon drop off each day if necessary
    -Do not bring anything from home such as toys, expensive electronics or anything that could be interpreted as dangerous or distracting to the activities or other campers. Any object deemed inappropriate, dangerous, or distracting will be taken and it will be left at our check-in desk and parents can pick them up at the end of the day.
    -Youth who bring their own cars are to turn in their keys to the Executive Director so that they are not lost, taken or damaged during activities and returned to the driver at the end of the day or in case they need to leave the premises ahead of the end of day and will park in the facilities parking lot.

    3. Imagination Station is a fun experience and everyone is to participate in the planned activities.
    - When you hear the change of station sound, go immediately to the next numbered station at the facility. (1 travels to 2, 2 to 3, etc.)
    - Be on time and ready to participate.
    - If ill, report it to an Imagination Station Leader immediately
    - Be a positive team member for your group and a positive role model for others
    - Please follow directions as they are given to you
    -If a Camper wishes to make a call, they can contact a Content Leader to make arrangements to call from the front desk area on their device (as that is where they will be stored).
    - Everyone must sign in/out with the Camp Director or staff at the front desk upon drop off or pickup.

    4. The following items and activities are not allowed at Wave of Hope Learning Centers Imagination Station Summer Camp (no second chances and fees are nonfundable). Staff will be dismissed immediately and Campers will be removed permanently.
    - No alcoholic beverages, knives, firearms, fireworks, illegal drugs, matches, and tobacco are allowed.
    - No gambling or betting with money, overly display of affection between anyone, fighting,
    threatening/physical abuse, stealing, tampering with emergency equipment, and being under the influence of drugs or alcohol are allowed at summer camp.

    CONSEQUENCES:

    Depending on the severity of the situation, Executive Director of Wave of Hope Learning Centers or Imagination Station Content Leaders may use the following steps (and may escalate steps depending on the situation) if a camper or adult staff member does not abide by the rules.

    1st Infraction: Discuss the inappropriate behavior with an adult staff member or teen counselor and clarify the rule.

    2nd Infraction: Camp Director staff will discuss the inappropriate behavior and give a "time out" or appropriate consequence. Camper's situation and/or behavior will be discussed.
    3rd Infraction or Any Behavior Listed in Rule # 4: Camp Director or staff will request that a parent/guardian come pick up a camper to be taken home and the summer camp fee of $150 per child will not be refunded. Adult Staff members will be asked to leave camp immediately.

    Additional consequences may be releasing the individual to the nearest law enforcement agency, assessing the cost of
    damages and repairs in the event of destruction of property, barring the individual from future Wave of Hope Ministries/Learning Centers/Enrichment activities in the future. Parents will be notified of any action taken.
